This form is for new members (not renewing members) who are buying a membership with cash, check, or Authorize.net (manual credit card payment)--fill it out, printing very clearly, and turn it in at the club along with the money, check, or printed receipt from Authorize.net. You don't need to fill out this form if you pay through the website membership software, which uses Paypal for credit card payment, or if you are renewing and paying at the clubhouse with cash, check, Authorize.net, or volunteer hours.
Required for youth memberships, sailboat rides for under 17.
